    The Core Difference: Volume vs. Position

    The single most important distinction to understand:

    • Breast augmentation addresses volume — it makes the breasts larger or fuller using implants or fat transfer
    • Breast lift (mastopexy) addresses position — it raises and reshapes breasts that have drooped or sagged

    You can have excellent volume but significant drooping. You can have good position but wish for more size. Or you can have both concerns — which is the most common scenario Dr. Gross encounters.


    What Is Breast Augmentation?

    Breast augmentation — also called augmentation mammoplasty — increases the size and fullness of the breasts using implants (saline or silicone) or, in some cases, the patient’s own fat through a fat transfer technique.

    Who Is Breast Augmentation For?

    • Women who have always had smaller breasts and want more fullness
    • Women who have lost breast volume after pregnancy, breastfeeding, or significant weight loss
    • Women who want to correct asymmetry between their breasts
    • Women whose breasts sit in an acceptable position but simply lack volume

    What It Does NOT Do

    Augmentation does not lift the breasts. If you have significant drooping (ptosis), adding an implant will make the breasts larger — but it will not move them higher on the chest wall or correct the sagging. In fact, adding volume to a sagging breast can sometimes make the droop appear more pronounced.

    What Is a Breast Lift?

    A breast lift (mastopexy) reshapes and elevates the breasts by removing excess skin, repositioning the nipple and areola to a higher position, and tightening the surrounding breast tissue. No implant is used unless the patient also wants added volume.

    Who Is a Breast Lift For?

    • Women whose breasts sag or droop, regardless of size
    • Women whose nipples point downward or fall below the breast crease
    • Women who have excess or stretched skin around the breasts
    • Women who have lost significant breast volume and position after pregnancy or major weight loss
    • Women who are satisfied with their breast size but not their position

    What It Does NOT Do

    A breast lift does not significantly change the size of the breasts. If you want to be noticeably larger, you will need augmentation as well. A lift also does not permanently stop the aging process — results are long-lasting, but the breasts will continue to change naturally over time.


    The Ptosis Scale: How Much Have Your Breasts Drooped?

    Surgeons use a grading system called the Regnault ptosis scale to classify the degree of breast drooping:

    Grade Description
    Grade I (Mild) Nipple is at the level of the breast crease
    Grade II (Moderate) Nipple is slightly below the breast crease
    Grade III (Severe) Nipple is well below the breast crease, pointing downward
    Pseudoptosis Nipple position is acceptable but the lower breast tissue droops below the crease
